Application for Congregation Status

The application from the mission or congregation must address the six requirements listed in Title IB, Canon 1, Sections 1- 3 before it can be recommended to the Standing Committee for approval. Please use the documents below to assist in submitting the application based on those requirements. Please contact the Rev.
